It is not unknown for a valve heater to do this - as a matter of fact, it's more common than we'd like to admit. Just a few weeks ago I had this happen in a KT88 output valve. I suspect that it's caused by a malfunctioning spot weld inside the valve.

Thank you Peter for your confirmation that this can happen. I really cleaned the wire inside the hollow pin and the inside of the pin, first with a small drill bit and then a needle file.I then re-soldered. I am confident there is a good electrical join between the wire and the base pin.
